Application for Appointment to a County Advisory Body

To apply to serve on a County advisory body, please complete the application below. To submit your application electronically, please click the yellow “Submit Application” button located at the end of the application form. To submit your application in hard copy form, please click the blue “Print Form” button located at the bottom of the application form. Hard copy applications can be hand delivered or submitted by mail to the Board of Supervisors, 701 Ocean Street, Room 500, Santa Cruz, CA 95060.

All sections with an asterisk (*) are required. It is not possible to save an application and submit at a later date.

Upon receipt, your application will be routed to each member of the Board of Supervisors and then filed for further consideration by Board members when there is a vacancy on the advisory body. If a Board member is interested in nominating you for appointment, you will be contacted to discuss the appointment, the appointment process, and requirements for the advisory body in question.

If you are interested in being considered for appointment to more than one advisory body, a separate application must be submitted for each appointment you are seeking.

Please note: This application is a public document and will be disclosed upon request with the demographic information omitted. In addition, copies of applications of those selected for appointment will be included in the Board's printed agenda packet.
